Background: Second-degree burns are among the most common cutaneous injuries, and appropriate wound dressing plays a critical role in accelerating healing, reducing pain, and improving clinical outcomes. We aimed to compare the therapeutic efficacy of Alloderm and Agi coat dressings in patients with second-degree burns not requiring skin grafting.
Methods: In this double-blind randomized clinical trial, 40 patients with second-degree burns admitted to Taleghani Hospital, Ahvaz, Iran, in 2025 were randomly enrolled to receive either Agi coat or Alloderm dressing. Pain intensity during dressing changes was assessed on days 4, 8, and 16 using the Visual Analog Scale (VAS). Wound healing rate, incidence of infection, duration to complete epithelialization, scar severity based on the Vancouver Scar Scale (VSS), and length of hospital stay were compared between the two groups.
Results: No significant difference was observed in the mean total burn surface area between the Agi coat and Alloderm groups (29.5% vs. 27.84%, P=0.07). Patients treated with Alloderm had a significantly shorter hospital stay compared with the Agi coat group (12.28 ± 2.49 vs. 16.70 ± 3.33 days, P=0.04). Pain scores were significantly lower in the Alloderm group on both day 4 (4.02 ± 2.30 vs. 6.36 ± 1.87, P<0.001) and day 8 (1.14 ± 1.21 vs. 2.82 ± 1.20, P<0.001). Complete epithelialization occurred faster in the Alloderm group (13.64 ± 3.16 vs. 16.04 ± 3.75 days, P=0.001). However, scar severity was significantly lower in the Agi coat group according to VSS scores (P<0.001). Infection rates were comparable between groups (P=0.372).
Conclusion: Alloderm dressing was associated with reduced pain, accelerated wound healing, and shorter hospitalization in second-degree burn patients, whereas Agi coat demonstrated superior scar quality outcomes.
